Our duties

Under the freedom of Information Act 2000 (FOI), Wallasey Village Dental Practice has two main duties; these are to have a publication scheme and to deal with individual requests for information.

Publication scheme

The following is a guide to information routinely available but is not a complete list of publications. This information is available on our website and/or paper version at reception or Anne O’Hare. The following is information routinely available

  • Contact details for the practice telephone number and opening hours
  • Arrangements for emergency out of hours care
  • The names of the dentists and hygienist who work here and their GDC numbers (we are registered and governed by the General Dental Council)
  • The types of treatment we offer and cost of treatment
  • Details of entitlement to exemption from NHS charges
  • Leaflets are available which explain clearly most of the treatments we carry out. Most of these leaflets are free
  • We are contracted by Wirral PCT to provide a limited amount of NHS dental treatment

Governance

We follow guidelines practices and procedures as advised by the GDC, Faculty of General Dental Practice, Wirral PCT, NHS and NICE. We are inspected by Wirral PCT, NHS BSA and CQC.

Personal Data

Personal data is protected by law under the Data Protection Act 1998 Only information necessary to carry out dental treatment safely, efficiently and effectively is collected. It is stored securely and destroyed after eleven years or in the case of children up to the age of twenty five years, whichever is the longer. The information is only shared with other health professionals to help provide care. It can be shared with other organisations such as Benefit Agency, Inland Revenue or when required by law or a court order. Patients have the right to access the personal information that we hold about them.

The service we offer

We will respond to a FOI request within twenty working days, requests must be in writing and addressed to A O’Hare stating the name of the applicant, and address for correspondence and a description of the information required. We charge £10 for computer held information and £50 for paper version of information.

We have a complaints procedure policy and any complaints about our service should be addressed to Complaints Manager, Wallasey Village Dental Practice, 39 Leasowe Road, Wallasey Village, Wallasey, CH45 8NY.
